Community Based Services
CCRSI offers flexible, person-centered Home and Community-Based Services (HCBS) that support individuals in living independently and actively participating in their communities. Our services are tailored to each person’s needs and focus on community inclusion, skill development, and meaningful relationships.
Community Networking (CN)
Community Networking (CN) provides one-to-one support to help individuals access and participate in community activities based on their personal interests, goals, and assessed needs. Services follow person-centered plans and focus on promoting independence, choice, and natural supports.
Supports may include:
- One-to-one assistance to participate in community activities and connect with community resources
- Support with building social skills, communication, and self-advocacy
- Support with developing and maintaining meaningful relationships
- Ongoing tracking of progress toward individualized outcomes
Community Networking – Group (CN Group)
Community Networking Group (CN Group) provides structured small-group support to promote socialization, community participation, and shared learning experiences while supporting individual goals.
Supports may include:
- Participation in community activities with peers who share similar interests
- Opportunities to build social skills, teamwork, and communication
- Shared activities that promote inclusion and community presence
- Staff support to encourage engagement and independence
Individualized Skills Development (ISD)
Individualized Skills Development (ISD) is a person-centered service that supports individuals in developing, improving, or maintaining skills needed for everyday life and long-term independence.
Supports may include:
- Skill development in daily living, self-care, decision-making, and problem-solving
- Support with money and time management, organization, and planning skills
- Building skills related to employment readiness and community participation
- One-to-one support focused on measurable outcomes, progress, and strategies that promote independence and confidence
Our goal with our Community Based Services is to integrate and connect our residential consumers with the greater community, establishing connections and building relationships and friendships while learning skills for life.

Tiered Supports enable agencies to tailor organizational activities and resources efficiently, including universal strategies supporting all individuals, specialized strategies supporting some individuals who need more support, as well as individual intensive strategies for those who may need it.
